Notarized Affidavit

A sworn affidavit is the traditional option of choice and the most recommended route when declaring that your child will be homeschooled. It is a notarized document that you will submit to your district with other documentation: yearly objectives, proof of vaccinations/religious or medical exemption, and proof of health and medical services.

Affidavits must be submitted by August 1st.

Annual Objectives

This document is included with your Notarized Affidavit.  The purpose of the objectives is to provide your goals for your child throughout the academic year.

Portfolio

"An eclectic documentation of life."

The portfolio (physical or digital) includes samples of work along with artifacts from field trips and other educational opportunities from throughout the academic year. Each portfolio must include a record of 180 days or 900 hours of instruction, a log of books and materials used, and proof of fire safety education.

Evaluators recommend at least three articles of documentation per subject (one each from the beginning, middle and end of the year). Artifacts can be documents, photos, videos, art projects, etc. When your portfolio is ready, it is time to schedule an evaluation.

End of Year Evaluation

An evaluator of your choosing with look at your portfolio including samples of work from the beginning, middle and end of year. Upon completion of the evaluation, you will receive a form to submit to your district.

Evaluations must be submitted by June 30th of each year.
